Waivers
To participate in skiing, snowboarding or snow tubing at Perfect North Slopes, participants should refer to waiver release forms, and complete the appropriate form or forms. Those participants under 18 who are not accompanied by a parent/guardian may electronically sign or print any of the following waivers and have a parent/guardian sign in advance.
Skiing / Snowboarding / Tubing Waiver
Participants under 18 who are not accompanied by a parent or legal guardian may electronically sign or print this and have their parent or legal guardian sign in advance. Please bring this document signed if you are participating with a group, participating in snow tubing, or purchasing a Season Pass of any type.
Rental Agreement
Please sign electronically or download and sign this document when renting ski equipment, snowboard equipment, and/or a helmet as part of participating in the recreational snow sports at Perfect North Slopes. This is to be used in conjunction with a signed Ski/Board/Tube Waiver also available on this page.
Ski/Snowboard Racing Program/Event Participation Agreement

Terrain Park Event Participation Agreement
Please electronically sign this document prior to participating in any terrain park event at Perfect North Slopes. Minor participants will be required to have their Agreement signed by a parent or legal guardian.
Carnival Event Participation Agreement
Please electronically sign this document prior to participating in any Carnival event at Perfect North Slopes. Minor participants will be required to have their Agreement signed by a parent or legal guardian.
Ski Train Depot
Please sign electronically or download and sign this document when renting take home ski equipment, or take home snowboard equipment from Ski Train Depot.
ColorFall Participation Agreement
Please electronically sign this document prior to participating in the ColorFall immersive experience at Perfect North Slopes. Minor participants will be required to have their Agreement signed by a parent or legal guardian.
